The "Catalog of Services" contains three sections: General Information, Medical Necessity, and the A-Z Test Listiing. Tests in this catalog are cross-referenced with synonyms referring the user to the corresponding test name and test code when appropriate. The Test Directory is searchable by Test Name, Test Code, and Keywords. Specimen requirements are provided in this section. When necessary, a reference is given in the test listing to the General Information section that contains more extensive instructions for selected assays.
General Information Information relating to services, policies, and operating procedures of Sanford Laboratories is included in this section. This section is also referenced for those tests requiring specific instructions and information beyond the "Specimen Requirements."
Medical Necessity This section contains detailed information on medical necessity and use of the Advanced Beneficiary Notice.
Test Directory - A-Z Test Listing The Test Directory search includes the comprehensive A-Z Test Listing. As an aid, CPT Coding is provided for informational purposes only. Sanford Laboratories assumes no responsibility or liability for the accuracy of CPT Codes published, as CPT Coding is the sole responsibility of the billing party. An option to view or print the Test Directory sections is available below the search area.

Individual Test Information - A-Z Test Lisitng

Each test listing provides: Test Code, Test Name, CPT Code(s), and Specimen Requirements/Reference Ranges.
Test Code and Test Name The unique test code assigned to the corresponding test name is listed. This section gives information pertaining to the test name, test methodology, and individual test components when applicable. Information useful in determining test selection is also provided.
CPT Codes The CPT code for a single test is aligned with the test name. When multiple CPT codes and tests are included, the CPT codes are given in the order of the tests listed. When a test group requires multiple CPT codes, the CPT code is aligned with the individual component of the test.
Sanford Laboratories assumes no responsibility or liability for the accuracy of CPT codes published. The CPT codes are provided for informational purposes only. This coding is based on our current understanding of Medicare rules and carrier implementation instructions at the time the codes were assigned, and is subject to changes without notice. Sanford Laboratories cannot accept responsibility for reimbursements you may or may not receive based on procedure coding we have provided. It is the client's responsibility to determine the appropriate coding when a client is billing a third party payor for services performed at Sanford Laboratories.
Specimen Requirements/Reference Ranges Generally, the Specimen Requirements format specifies the requested volume, specimen type, minimum volume, storage temperature, and special handling notes. The requested volume is the amount sufficient to allow multiple runs of the assay if needed.
The minimum volume allows for one assay run with allowance for sample loss when specimen is aliquoted.
Storage temperature is specified as room temperature (18-22ºC), refrigerated (2-6ºC), or frozen (-20 Cº or colder). When temperature is not indicated, the sample may be stored and shipped in the most convenient manner for the client.
Specific instructions are given regarding patient preparation and collection of the specimen, proper specimen container, shipping instructions, and causes for rejection of the specimen by the laboratory. Specimen stability is provided for many of the tests; for example, RMT = room temperature, REFT = refrigerate.
Frozen specimens: If multiple tests are ordered requiring a frozen specimen, EACH TEST REQUIRES ITS OWN FROZEN ALIQUOT. Individual FROZEN ALIQUOTS will help ensure that the specimen remains at optimal stability for testing and will help reduce the risk of test cancelations.
The Reference Range portion contains the expected reference range of the test, along with interpretive data and guidelines when appropriate. PLEASE NOTE: Reference ranges are subject to change with the current reference range provided on the patient's test report form.
